Die Gültigkeit und Nützlichkeit psychiatrischer Diagnosen wird seit über einem Jahrzehnt konsequent kritisiert. Die Validität der Posttraumatischen Belastungsstörung (PTBS) wird aufgrund der Unspezifität des Syndroms, der Unspezifität der Symptome und der theoretisch angenommenen Monokausalität infrage gestellt. Bedeutende Kritik betrifft die hohen Komorbiditätsraten mit anderen psychiatrischen Störungen. Dem Phänomen der Komorbidität liegen philosophische und konzeptuell-methodische Probleme zugrunde, die auf der Wahl des theoretischen Modells zur Erklärung von psychiatrischen Störungen beruhen. Die vorliegende Arbeit stellt eine narrative Literaturübersicht dar. Zur Gewinnung der Literatur wurde eine Computerrecherche in der wissenschaftlich-medizinischen Datenbank PubMed durchgeführt. Die Schwerpunkte der Arbeit wurden auf die Validität und Komorbidität von PTBS gesetzt: Zum einen wird die PTBS-Diagnose hinsichtlich ihrer Konstruktvalidität untersucht; zum anderen wird versucht, Aufschluss über die theoretische Konzeptualisierung des Syndroms und die Komorbiditätsproblematik zu geben. Die Übersichtsarbeit zeigt, dass die PTBS-Diagnose weiterhin mit Problemen und Kontroversen hinsichtlich der Validität verknüpft ist. Erste empirische Befunde zur taxonomischen Struktur der geplanten ICD-11 sprechen für eine Verbesserung hinsichtlich der Gültigkeit und des klinischen Nutzens der Diagnose. Zur semantischen Interpretation bestehender Konzepte von psychischer Störung wurden drei Positionen diskutiert: Konstruktivismus, Realismus und Kausalsystemansatz. Anstatt PTBS als natürliche Kategorie oder soziale Konstruktion zu konzipieren, kann es wissenschaftlich nützlicher sein, PTBS als umfassendes kausales System von möglichen traumabezogenen Reaktionen zu betrachten und zu prüfen. Durch eine dynamische Herangehensweise und vielzählige Analysemöglichkeiten können Untersuchungen zur Netzwerkstruktur von PTBS eine nützliche Methode darstellen, um einen genaueren Einblick in die komplexe Psychopathologie der PTBS zu gewinnen.

The validity and usefulness of psychiatric diagnosis has been criticized consistently for over a decade. The validity of posttraumatic stress disorder (PTSD) has been questioned due to the non-specificity of the syndrome, as well as the symptoms and the theoretically assumed monocausality. Major criticism results from the high comorbidity rates with other psychiatric disorders. Philosophical and conceptual-methodological issues that are based on the choice of the theoretical models to explain psychiatric disorders, underlie the phenomenon of comorbidity. The present work presents a narrative review of the literature, which was obtained by conducting a computer search in the scientific and medical database, PubMed. The work has two main focuses. First, the PTSD diagnosis is examined in terms of its construct validity. On the other hand, the paper tries to shed light on the theoretical conceptualization of the syndrome and the problem of comorbidity. This review article shows that the PTSD diagnosis is still associated with problems and controversies concerning the validity. First empirical evidence on the taxonomic structure of the proposed ICD-11 points out improvements in the validity and clinical utility of the diagnosis. For the semantic interpretation of existing concepts of mental disorder, three positions were discussed: constructivism, realism and causal system approach. Instead of conceiving PTSD as a natural category or social construction, it can be scientifically beneficial to look at and consider PTSD as a comprehensive causal system of possible trauma-related reactions. The dynamic view of research on the network structure of PTSD can be a useful way to gain more insight into the complex psychopathology.
